We present a series of 11 cases that associated increased sagittal diameter in a transparent lens, which induced the condition. A clear lens in a relatively young patient (fourth-fifth decade of life) can obscure its involvement in the disease mechanism. Patients presented after a poor response to peripheral laser iridotomy with high intraocular pressure (IOP), glaucomatous optic neuropathy, a clear lens and mild refractive changes. All patients were investigated thoroughly including biometry and ultrabiomicroscopy (UBM) with anterior chamber depth and lens measurements. After establishing the importance of the lens in the pathogenesis of this disease we performed phacoemulsification with a positive outcome in all cases, confirming once again our theory. We considered this new entity occult phacomorphic glaucoma in order to differentiate it from the one associated to advanced cataract and to highlight the fact that there are no lens induced myopia or transparence changes. Lens involvement is obscured by its transparency and can be documented by UBM.

Retrospective analysis of the risk factors for developing phacomorphic glaucoma

AIM To determine the risk factors for developing phacomorphic glaucoma in eyes with mature cataracts. MATERIALS AND METHODS This is a case-control study comprising of 90 eyes with phacomorphic glaucoma and 90 age- and sex-matched control eyes with mature cataracts without phacomorphic glaucoma. Patients with pre-existing glaucoma, previous intraocular surgery and /or absence of documented axi...

An Evidence-Based Approach to Phacomorphic Glaucoma

Cataract maturation is associated with anteroposterior lens diameter increase which, in some patients, particularly those with anatomically narrow anterior chambers, can lead to pupillary block and peripheral iridotrabecular apposition [1,2]. In other patients with previously deep anterior chambers and wide open angles, the cataract may become so swollen that it physically pushes the iris anter...

Visual Outcomes Following Manual Small Incision Cataract Surgery for Phacomorphic Glaucoma

Aims: To evaluate intraocular pressure (IOP) control, visual prognosis and complications following manual small incision cataract surgery among eyes with phacomorphic glaucoma. Material and Methods: This prospective, non-randomized interventional consecutive case series included 10 eyes of 10 patients with phacomorphic glaucoma who presented to Kasr El-Aini Hospital between March 2010 and April...

Phacomorphic Glaucoma: Evolving Management Strategies

Phacomorphic glaucoma is defined as secondary angle-closure glaucoma due to an increase in lens thickness and is characterized by a sudden rise in IOP which can compromise the function of the optic nerve and may lead to irreversible visual loss if not treated in time. This increase in lens thickness can be due to an advanced cataract, a rapidly intumescent lens, or a traumatic cataract, eventua...

The Use of Femtosecond Laser-Assisted Capsulotomy Is Challenging in Patients With Phacomorphic Glaucoma.

To the Editor: Acute phacomorphic glaucoma occurs secondarily to an increased anteroposterior lens thickness that narrows the iridocorneal angle. Optic neuropathy is preventable with early recognition and treatment of the disease, but a sudden rise in intraocular pressure (IOP) can lead to irreversible visual loss in the affected eye.1-3 We evaluated the efficacy of femtosecond laser–assisted c...
